Keeping your name accurate on Maple is important because it appears on prescriptions and notes from practitioners. You can also add a preferred name for communications.
Once you've completed a visit on Maple, you'll no longer be able to edit the name on your account. This is because your name has been verified and used to create official medical documents, including:
Prescriptions
Lab requisitions
Practitioner's notes
Visit summaries

Want to change the name of a dependent on your account? Click here.
Changing the account holder's name
Log in to Maple and go to Settings > Account.
Change your name in the fields provided.
If these fields are grayed out, your name has already been verified and can't be changed in your Account settings. You'll need to contact our Customer Support Team with a government-issued photo ID to update it.
Click Save to confirm your changes.
