It's important that your profile information matches your ID. This ensures you can fill any prescriptions or lab requisitions issued through Maple. You can also use the steps below to update the name of a dependent on your account.
Once a visit has been completed, patient names can't be edited. This is because we've verified your name and used it to create official medical documents, including prescriptions, lab requisitions, doctor's notes and visit summaries. If your profile information is incorrect, please contact our Customer Support Team for help.
How to update your profile information
Go to Profiles in the main menu.
Select a patient profile you want to update.
Click Edit personal profile.
Update the information as needed and click Save changes.
If you are unable to save your changes, you'll need to contact our Customer Support Team with a government-issued photo ID to complete this update.
