Adding medical records to your Health profile

Step-by-step instructions for adding new or correcting existing information in your health profile

Filling your health profile (such as identification, allergies, past immunizations, or medications) can help our physicians provide the best diagnoses and treatments.

Your health profile can be updated in two places: in your Health hub, or during a consultation (either while you wait for it to start or once you speak with the physician).

Select an option below to jump to the appropriate section:


Uploading medical records to profile

  1. In the main menu, select Profiles. If applicable, select the patient you wish to edit a record for.

  2. Scroll down until you reach the option you would like to use. There is a button to add an assessment, and a button to add a test. Select whichever option applies to you.

  3. To add an assessment result, fill in the text fields as applicable and click Save Changes.

  4. To add a medical test, fill in the text fields as applicable and click Add test.

  5. You may also upload a file or image to the medical test by selecting Upload files.

  6. Select an upload option to add a file from your device.

Revise or delete a medical record

  1. In your health profile, scroll down to locate the test or assessment you want to edit, and select the one you wish to change.

  2. Select either:

    • Edit — to revise information.

    • Delete — to remove the record entirely.

    Locate and select the test or assessment you want to edit
