Adding medical records to your Health profile

Step-by-step instructions for adding new or correcting existing information in your health profile

Filling your health profile (such as identification, allergies, past immunizations, or medications) can help our physicians provide the best diagnoses and treatments.

Your health profile can be updated in two places: in your Patient profile, or during a consultation (either while you wait for it to start or once you speak with the physician).

Select an option below to jump to the appropriate section:


Adding medical records to profile

  1. In the main menu, select Profiles. If applicable, select the patient you wish to edit a record for.

  2. Under Key medical records, select a category to add your information to.

  3. Fill out the details of your record, and click Save.

  4. Your new record will now be visible under the category you selected. To edit or add uploads to it, click on the record.

    1. To attach an upload, click Upload files. To update the information you added, click Edit.

In the main menu, select Profiles.

Edit or delete a medical record

  1. In your health profile, select the category and condition you want to edit.

  2. Select either:

    • Edit — to revise information.

    • Delete — to remove the record entirely.

    • Hide this record from providers

  3. To delete an upload, click on the upload and then click the Trash icon at the bottom of the view.

Select the test or assessment you want to edit

Select the option that applies to you: Edit, Delete, or Hide this record from providers.

To delete an upload, click on the upload and then click the Delete icon.
