Yes, you can add a preferred name and gender information to your account at any time.

Setting your name on your account

We ask for your legal name as well as your preferred name. Our team will refer to you by your preferred name in our communications.

For medical documents to be valid, the legal first name on the account must match the name on your government-issued photo ID. This is required by pharmacies for prescriptions, as well as labs and clinics for any tests ordered. You will see both names listed on any medical documents issued by Maple doctors.

Add a preferred name to your Maple account anytime by logging in and clicking on Settings from the left navigation menu.

Setting your gender identity and pronouns

We currently ask that everyone creating a Maple account fill in the sex assigned at birth field — if you are not comfortable disclosing this, please select “Prefer not to say”. Your sex assigned at birth plays an important role in your unique disease risk factors. Our doctors will only ask for this if it's relevant to your medical concern.

Add your gender identity and pronouns to your Maple account anytime by logging in and clicking on Patient Profiles in the left navigation menu.

You can also indicate your gender identity and pronouns. This helps our team communicate with you.
