A lab requisition is a document a practitioner provides after your visit. It authorizes specific lab tests — like blood work or urine tests — to help them get a clearer picture of what's going on and recommend the right treatment. You can bring your lab requisition to a local lab of your choice.

Depending on your province of residence, diagnostic imaging may be available.

Who can get a lab requisition on Maple

Lab requisitions are available in most provinces for those with Maple memberships and patients who have certain coverage through employer benefits or private insurance plans.

In Nova Scotia, only patients on the VirtualCareNS Full Care program will have access to lab requisitions. To learn more, click here.

All medical care—including requisition, prescriptions and treatment directions—is at the practitioner's discretion. The practitioner is also required to verify your identity before issuing a requisition.
