Lab or diagnostic imaging (DI) requisitions are available through a consultation with a doctor or nurse practitioner on Maple, depending on your location and coverage. Click here to see if this service is available in your province.
Receiving a requisition
When a healthcare provider orders a test for you on Maple, you’ll receive a PDF requisition that you can print and take to any community lab or diagnostic imaging centre.
Please note that, depending on the tests ordered, the centre may charge additional fees. These are determined by the centre, not Maple. If the lab/DI centre needs additional information, they may either contact our team, or directly contact the ordering provider.
After your tests are completed, the results are sent back to Maple. You can learn how to access your results here.
Steps to access your requisition
From the main menu, click Requisitions.
Locate and download the requisition form. It may also be visible on your dashboard under recent history.
If you do not find your requisition here, check the Uploads section of your consultation.
Open the PDF on a computer using Preview or Adobe Reader.
Print out the form and fill in any additional information, or fill it out on your computer.
Take the printed form to a lab of your choice or email it to them.
In the main menu, click Requisitions.
If you do not find it under Requisitions, check the Uploads section of your consultation.
