Practitioners may provide a sick note if you requested one in your visit submission. It's always at their discretion.
After your Maple visit, you can download a copy of your practitioner's note for work, school or personal records.
How to download your practitioner's note
Go to the Maple website and sign in to your account.
Go to the Consultations section on your dashboard or click the three horizontal lines at the top-right corner.
Select the consultation and scroll down to the Details section.
Click Notes.
Click Download.
