About medical notes
Need a sick note or doctor's note? Doctors and nurse practitioners on Maple can issue medical notes when appropriate—just let them know when you submit your visit request. Include any relevant details or supporting information to help your practitioner understand what you need.
Medical notes are PDF documents that practitioners can add to your visit on Maple. The note includes the practitioner's name, licence number, Maple's contact information, and your name, date of birth, and phone number. Medical notes don't share symptoms, diagnosis, or other personal health information.
Your practitioner will review your situation and decide which type of note best fits your needs. Maple issues three kinds of notes:
Practitioner-certified absence notes confirm you're unable to attend work or school.
Visit confirmation notes document your visit.
Non-absence notes are used for accommodations, referrals, or other documentation.
Medical notes aren't guaranteed—it's always at the practitioner's discretion.

There's no extra cost for medical notes on Maple.
Certified absence notes
Your workplace or school may require specific information in a medical note. You should always check with them to understand what’s required. Your practitioner will use their clinical judgment to determine whether they can issue a note that fits with these needs.
Keep these limitations in mind when asking for a certified absence note:
You must have a video call with the practitioner to get a certified absence note
The start date of the absence period can't be more than two days before your visit
The absence period can't be more than seven consecutive days — including back-to-back notes issued separately
No more than six certified absence notes can be issued within a 12-month period
Medical note verification
If your school or employer contacts Maple to inquire about a note you received, we'll confirm the date of your visit and let them know if a note was issued. Note: Any medical notes issued before June 2026 may be categorized as either absence notes or custom notes.
