This article only applies if you are in Ontario. If you are outside Ontario, please see our article on photo ID, available here.

Verifying your identity on Maple with a valid government-issued photo ID helps us ensure your account details are accurate so you can enjoy a smoother experience.

When you submit a consultation request for a general practitioner, you'll encounter the Verify your identity screen. To continue with your consultation request:

  1. Select the type of ID you have available:

    1. Photo health card

    2. Non-photo health card

    3. I don’t have a health card

  2. Take or upload clear photos of the front and back of your government-issued ID. Make sure the images are well-lit and all information is clearly visible.

Once your ID is successfully verified, you'll proceed directly to the consultation waiting room.

What to do if the verification fails

Don't worry if we can't verify your identity automatically — you'll still be able to proceed with your consultation. The healthcare provider you're matched with will manually verify your ID during your appointment.
