Verifying your ID in the Maple app

This article only applies if you are in Ontario. If you are outside Ontario, please see our article on photo ID, available here.

Verifying your identity with a valid government-issued photo ID keeps your account accurate and your Maple experience running smoothly.

When you submit a visit request to see a doctor or nurse practitioner, we'll ask you to verify your ID.

  1. Click Verify ID.

  2. On the next page, select the type of ID you have available:

    • Photo health card

    • I don't have a health card

  3. Click Continue.

  4. You'll land on the Getting started page—click Begin verifying to upload your ID.

  5. Take or upload clear photos of the front and back of your government-issued ID. Make sure the images are well-lit and all information is clearly visible.

Once we've verified your ID, you'll move on to the waiting room.

What to do if the verification fails

Don't worry if we can't verify your identity automatically — you'll still be able to proceed with your visit. The practitioner you're matched with will manually verify your ID when your visit begins.
