Adding insurance policy numbers to your account

Depending on your coverage, you may be asked to share insurance policy numbers.

When you're adding your health coverage to your Maple account, we'll ask for details like your insurance policy number or other employee information. This helps us confirm your coverage and get you connected to care.

Your policy or certificate number is a unique code that identifies your insurance plan. You'll find it on your insurance card or other insurance documentation. To get your policy number, contact your insurance provider — they'll have the most up-to-date information about your coverage and plan ID.

If you see an error after entering your policy number, contact customer support for help.
