If the policyholder is under 14 years of age, their homestay parent/guardian will need to create their own account and add the policyholder as a dependent. As a homestay parent/guardian of an insured minor (<14 years old), please follow the steps below to create a Maple account on their behalf:

  1. Visit getmaple.ca/msh to get started.

  2. Enter the MSH policy number and DOB for the minor in your care. Uncheck “plan information belongs to me” and proceed to register for an account in your name

  3. Create the Maple account using the homestay parent's details (name and birthdate).

  4. Apply account coverage to minor in your care. Click “Patient Profiles” from the dashboard and add the minor as a patient. Then, in “Account Settings” under the “Coverage” tab, re-enter the minors policy number and DOB to apply their coverage

  5. The insured minor can now access their Maple coverage and connect with a doctor on the platform using their covered visits.
