You will be prompted to enter your first name, last name, date of birth, Medavie identification number (11 digits) and policy number (10 digits), which must match exactly what is on your benefits card or in your Medavie account.
McGill students have a specific number format requirement. Your certificate/ID number is your McGill ID + 01.
Your McGill ID should be 9 digits, so if your McGill ID is 123456789, then your certificate/ID number is 12345678901.
Students enrolled in IHI before May 4th, 2019 add 00 to the end of their McGill ID instead of 01.
Your contract/policy number follows the format 00 + "12345" + 000. Your 10-digit contract/policy number should look something like 0012345000.
