To add your health coverage to your Maple account, you might need to enter details like your insurance policy number or other employee info.

A policy or certificate number is a unique code associated with your insurance plan. It serves as a distinct identifier for your coverage. It may be on an insurance card or other insurance documentation. To access your policy number, please contact your insurance provider, as they will have the most up-to-date information regarding your coverage and plan ID.
