Accessing your lab or imaging requisition

Please note that Maple is not intended for urgent testing, and all medical decisions–including the provision of requisitions–are at the practitioner's discretion.

Lab or diagnostic imaging (DI) requisitions are available through a visit with a doctor or nurse practitioner on Maple, depending on your location and coverage. To see if this service is available in your province, click here.

Receiving a requisition

When a practitioner orders a test for you on Maple, you’ll receive a PDF requisition that you can print and take to any community lab or diagnostic imaging centre.

Keep in mind that, depending on the tests ordered, the centre may charge additional fees. These are determined by the centre, not Maple. If the lab or DI centre needs additional information, they may contact our team or the ordering practitioner directly.

After your tests are completed, the results are sent back to Maple. You can learn how to access your results here.

Steps to access your requisition

  1. From the main menu, click Requisitions.

  2. Locate and download the requisition form. It may also be visible on your dashboard under recent history.

    1. If you do not find your requisition here, check the Uploads section of your consultation. Paramedical referrals are often located here.

  3. Open the PDF on a computer using Preview or Adobe Reader.

  4. Print out the form and fill in any additional information, or fill it out on your computer.

  5. Take the printed form to a lab of your choice or email it to them.
