Information regarding obtaining a doctor’s note on Maple

If you need a sick note (also called a doctor’s note or medical note), you can request one as part of your consultation.

Sick note limitations

The decision to issue a sick note is always at the discretion of the healthcare provider. Please be aware of the following limitations when asking for a sick note on Maple:

  • Backdated notes: Notes can only be issued for medical events within the past two days.

  • Notes for extended periods of time: Requests for notes lasting more than seven days may require an in-person assessment by a healthcare provider familiar with your medical history.

  • Recurrent notes: You may need to seek in-person care for recurring health concerns. For example, if you’ve had six or more notes within the past 12 months, a healthcare provider may advise you to see an in-person clinician.

  • Hidden notes: If you’ve hidden previous consultations with sick notes, providers may be unable to review your medical history to assess safety and determine whether virtual care is appropriate, which may prevent them from issuing a new sick note.

Sick note fees

Your Maple membership includes doctor’s notes at no extra cost. If you’re in Quebec and paying per consultation, the cost of the note is already included in your consultation fee.
