During your visit, if your practitioner determines your concern can't be treated virtually, they may recommend an in-person assessment (IPA). It happens — and when it does, we'll help you figure out what comes next.

For everyone else: Your practitioner will upload an in-person assessment form directly to your visit. It includes a summary of your Maple visit and information on local clinics — use it to contact a clinic and book your in-person appointment.
Finding your IPA form
From the main menu, click Consultations.
Select the visit that the form was issued in.
Click Uploads.
