A specialist referral means your care is moving forward. You'll need to accept the referral and share some information—like your mailing address—so we can find a specialist clinic near you.
Once you submit the form, you won't be able to edit it. If you need to make a change after that, please contact our Customer Support Team.
After we send your referral to a specialist, we'll keep you updated. We'll add the specialist's name, location and phone number to your referral, and they'll reach out directly to book your appointment. If that clinic can't take the referral, we'll find another one.
How to accept your specialist referral
Click Specialist Referrals in the main menu.
Click View Details.
Enter the required information, then click Submit.
