Follow-up questions or information for the provider after a consultation

If you have a quick question, or the provider you spoke to asked you to send in more information, you can upload a file to the completed consultation — simply follow the process below.

Please note that if you wish to discuss new or additional medical information, you would need to complete another consultation with a doctor to address any issues that were not discussed in your original visit.

Uploading files on web browser

  1. Log in to Maple and click Consultations on the left-hand menu. Select the consultation you would like to add information to.

  2. In your consultation, click the tab called Uploads.

  3. Click on the paper clip icon and add a file with your information.

  4. Ask a Maple customer service agent to inform the provider, by reaching out to us at the contact button below.

Uploading files on mobile

  1. Log in to Maple and click on the hamburger menu on the top left of the screen.

  2. Click Consultations, and select the consultation you would like to add information to.

  3. In your consultation, click Uploads.

  4. Click Upload a file and add a file with your information.

  5. Ask a Maple customer service agent to inform the provider, by reaching out to us at the contact button below.

Once you have informed a customer support representative, they will notify the provider that they have a message to review.
