Our physicians can provide doctor’s notes, at their discretion.

Request a doctor’s note at any time by clicking on Get Care in your Maple account. Be sure to let your physician know who the note is for and which dates.

It is at the doctor's medical discretion to determine the duration of the sick notes, as well as how far they can backdate them.

You'll get a digital copy (PDF) of the sick note which you can use for work or school. The doctor's signature and direct contact information is on the note if your school or employer has any questions.
