Supporting statements are short questionnaires completed by people who know you well. Your practitioner reviews them alongside everything else gathered during your ADHD Assessment to help confirm a diagnosis.
Who you'll need to invite depends on your program:
Adults: one statement from someone who knew you in childhood, and one from someone who knows you in adulthood. If a childhood contact isn't available, you can complete the questionnaire yourself — the adulthood statement is still required.
Paediatrics (ages 6–17): two statements from non-home observers, such as teachers or coaches.
Supporting statements are due before the 48-hour deadline for your Diagnosis and Treatment Plan visit, along with your other pre-visit steps. If this step isn't completed on time, your visit will be automatically cancelled. Send your invites as soon as this step appears to give your contacts plenty of time to respond.
How to send supporting statement forms to your contacts
Once you book your Diagnosis and Treatment Plan visit, supporting statements automatically appear as a required step in your Beyond ADHD program hub — no separate email or download needed.
Here's how to send them to your contacts:
On the Maple dashboard, under Upcoming visits, click on your upcoming Step 2: ADHD diagnoses and treatment appointment.
Under Before your visit>Supporting Statements, click Continue.
Click Invite People>Add a contact.
Enter the contact details and click Send invite.
Your contact receives a secure link by email — no login or Maple account needed on their end. They complete a short form, generally in 10 to 20 minutes.
You can see who's been invited, who's still pending, and who has submitted, right from your program hub.
Once a contact submits their statement, it can't be edited. If you need to make a change beforehand, you can cancel a pending invite and send a new one to a different contact.
