As a parent of an insured minor (<18 years old), please follow the steps below to create a Maple account on their behalf:
Visit getmaple.ca/ivari to get started.
Enter the policy number of the minor, the minor's date of birth, and check “Plan information belongs to me”. Then, click “Continue” to register for an account.
Create the Maple account using the details (name and birthdate) of the parent.
Once the account is created, click on “Patient Profiles” on the main dashboard, and add the insured minor as a patient.
The insured minor can now access their Maple coverage and connect with a doctor on the platform using their covered visits.
