Filling your health profile (such as identification, allergies, past immunizations, or medications) can help our physicians provide the best diagnoses and treatments.
Your health profile can be updated in two places: in your Health hub, or during a consultation (either while you wait for it to start or once you speak with the physician).

Web browser — Uploading medical records to profile
Log in to your Maple account here: https://app.getmaple.ca/login.
Click on Health Hub from the left side menu. If applicable, select the patient you wish to upload a record for.
Select Manage health profile.
In the patient's health profile, scroll down until you reach the option you would like to use. There is a button to add an assessment, and a button to add a test. Select whichever option applies to you.
To add an assessment result, fill in the text fields as applicable and click Save Changes.
To add a medical test, fill in the text fields as applicable and click Add test.
You may also upload a file or image to the medical test by clicking the paper clip icon. Simply drag and drop a file, or click to browse your computer to select a file. When done, click Save File.

In your health profile, click the option that applies to you

Web browser — Revise or delete a medical record
In your health profile, scroll down to locate the test or assessment you want to edit, and select the one you wish to change.
Select either:
Edit — to revise information.
Delete — to remove the record entirely.
In your health profile, select the test or assessment you added. You can edit or delete it using the buttons.

1. Log into your Maple app and click on the three bars in the top-left hand corner of the screen.
2. Select Profiles. If applicable, select the patient you wish to edit a record for.
3. Scroll down until you reach the option you would like to use. There is a button to add an assessment, and a button to add a test. Select whichever option applies to you.
4. To add an assessment result, fill in the text fields as applicable and click Save Changes.
5. To add a medical test, fill in the text fields as applicable and click Add test.
6. You may also upload a file or image to the medical test by selecting Upload files.
7. Select an upload option to add a file from your device.
