If you are eligible for a visit with an Endocrinologist through Maple, follow the steps below.
Requesting your initial Endocrinology appointment
Follow the main steps to request an on-demand consultation. See article: Steps to see a Maple provider as soon as possible.
You will receive an email instructing you to upload supporting documents, such as test reports or images, to your consultation.
The endocrinologist will review your request and begin the visit within 24 hours (or decline it, if you appear ineligible).
When you submit a consultation request on Maple, always select the patient profile for the person seeking care. At the account holder’s discretion, they can give the patient privacy with the provider once they have provided access to their Maple account.
