Lab requisitions are available through a consultation with a doctor on Maple, at their discretion, depending on your location and the type of test:
Blood work lab requisitions are available in all provinces, except New Brunswick, Nova Scotia, and the territories.
Diagnostic imaging lab requisitions are available in Ontario, British Columbia, Quebec, PEI, and Alberta.
